Description

The Outreach Worker is responsible for overseeing day-to-day, practical and supportive services offered at Hope Links North Perth.   The Outreach Worker will adopt the role of advocate, supporting client-identified goals, assisting with resource navigation, and providing case by case assistance.  The Outreach Worker will develop a short term, timely advocacy role in addition to hands on program duties, with the intention of improving quality of life for clients with persistent needs who are not eligible to participate in Pathway of Hope. The Outreach Worker will work as part of the larger Community Services (CS) team in providing short term programming as determined by the Community and Family Services Director in collaboration Community Services staff.

KEY RESPONSIBILITIES:

Operations/Program

  • Actively promote a positive environment in program and with clients, ensuring volunteers and clients are treated with dignity, courtesy, and respect.
  • Ensure clients and volunteers are informed of and adhere to policies, protocols and expectations contained in program related guidelines and TSA Policies and Procedures.
  • Maintain accurate records of attendance, volunteer participation, incident, and referrals and ensure each guest completes a program orientation and ongoing documentation using Link 2 Feed.
  • Demonstrate work methods and monitor volunteers, working to achieve the purpose of the program and of the organization.
  • Monitor the program environment/clients to ensure safety and security during program.
  • Perform internal and external security checks at the beginning and end of each shift.
  • Liaise with other community agencies to provide wrap around support as needed.
  • Regularly attend to self-care through appropriate de-briefing with Outreach Coordinator.
  • Ensure Hope Links program space is clean, and any maintenance issues are reported to the Outreach Coordinator as soon as possible.
  • Responsible for picking up program food daily from the church location and transporting it to the program site.

Case Management

  • Develop and maintain a short term, limited number case load.
  • In consultation with the Outreach Coordinator and the client, design and implement a plan to assist clients in reaching self-identified goals.
  • Assist clients in connecting to local services and agencies while supporting with resource navigation.

Health and Safety

  • Responsible for the following: work in compliance with the Ontario Health & Safety Act and Regulations, use personal prescribed safety equipment and clothing as directed by the employer, report workplace hazards, dangers, injuries or illness, work in a manner as required by the employer, and report workplace injuries or illness.
  • Work in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures.

WORKING CONDITIONS

  • This is a regular part time position of 32 hours per week.
  • There is some flexibility in scheduling according to case management needs, and program possibilities.
  • The working environment is typically at 295 Main St. West Listowel with some flexibility on non-program days.
  • Travel of 10% associated with this position to provide support and advocacy for a client attending a housing or banking appointment.
  • Required to drive a Salvation Army program vehicle when available.
  • Must be able to lift/move up to 40 lbs.
  • This job requires reaching, bending, and stooping frequently, working in a standing position for periods of time.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Education, Qualifications and Certifications:

  • Completion of a Community College Diploma in Social Services or equivalent field of study of two academic years.
  • Current, valid First Aid/CPR Certifications and Naloxone training required
  • Experience and Skilled Knowledge Requirements
  • Minimum of two (2) years of prior related experience, including, experience working with multi-barriered persons.
  • Knowledge of community-based resources.
  • Valid Ontario class “G” Driver’s License, own vehicle and insurance, and a current original copy of driver’s abstract that is satisfactory to The Salvation Army, in its sole discretion, is required.
  • Develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities.
  • The ability to relate to and engage with marginalized individuals.
