Sunnybrook’s Nurse-Led Outreach Team (NLOT) delivers specialized services including urgent in-home assessments, IV therapy, and ongoing staff education — helping reduce unnecessary emergency visits while supporting the dignity and comfort of our seniors.

With direct links to Sunnybrook’s outpatient and urgent care services, this collaboration is redefining what integrated, compassionate care looks like in long-term care settings.
