Position Purpose Summary:

Housing Retention Case Managers ensure housing stability through landlord engagement and subsequently connects York Region Residents with Low and Moderate Income who are experiencing homelessness or are at risk of homelessness with prescreened housing choices and landlords that enable housing stability. Housing Retention Case Management provides specialized, in-depth support which includes case planning that will focus on assessing the complexity and range of a client’s needs, within the framework of supports required to maintain housing supports will include  but  are  not  limited  to  eviction  prevention  supports,  system  navigation, warm transfer referrals, landlord tenant act training, rapid rehousing and housing based case management that will contribute to successful housing stability.

 

Responsibilities

Program Oversight:

  • Providing initial and ongoing assessments to determine needs and goals of clients, including the coordination of multiple services and service providers, case conferences, goal setting and referrals to other supports and services
  • Developing individualized case plans, personal goals, tenancy skills, employment/income, health and well-being to promote long-term housing stability
  • Housing-based case management supports and services to keep or find permanent housing
  • Provide clients with warm transfers to services such as addiction or budget / credit counselling, or enter into a trusteeship program to receive financial assistance
  • Provide system navigation support and warm transfers to access services and programs at agencies in York Region that meet individual needs and preferences for income, social, emotional, medical, and poverty relief supports to prevent recurring housing instability
  • Research, compile and maintain housing options, document acceptance criteria and procedures
  • Provide mediation support with landlords, utility companies, and other organizations, to ensure housing stability and / or rapid rehousing
  • Supporting clients with community integration and belonging, through meaningful community-based activities and opportunities
  • Assisting clients with developing social and required life skills, to secure and maintain permanent housing
  • Provide rapid rehousing support with clients as needed, to divert from emergency housing
  • Liaise with and make regular, ongoing contact with Landlords concerning possible vacancies
  • Provide statistical reports of number of apartment searches, placements, etc.
  • Create Service Agreements with Landlords
  • Facilitate housing assessment specific to participant to provide the most appropriate support/housing options
  • Provide time limited housing based intensive case management.
  • Work with clients to ensure they understand and fulfill their responsibilities as tenants
  • Assist Participants in meeting their financial obligations in regard to rent, bill payments (budgeting purposes) by defining individual responsibilities; resolving any financial disputes and any other financial responsibilities.
  • Meet with participants and/or partner agency case managers to provide guidance and information regarding all available housing stability options
  • Assist participants with applications for, and liaising with, Rent Geared to Income Housing
  • On an ongoing basis, assist participants with social and required life skills to secure and maintain affordable housing
  • Help participants to establish or re-establish themselves in the community
  • Provide landlord mediation and crisis intervention
  • Perform any other related duties as requested by Program
  • Travel to meet clients at multiple locations to better cater to clients needs and increase accessibility

Administrative Responsibilities:

  • Prepare case notes (intermittently throughout each working day) and share them via HIFIS to relevant individuals involved in the participant ’s circle of care while maintaining participant ’s confidentiality
  • Ensure that the privacy and confidentiality of participants is maintained within established standards, Policy and Procedure
  • Work as a member of the team to ensure that the program operates efficiently and effectively (and to ensure that other staff's responsibilities are covered when they are away)
  • Participate in and represent the organization at external committees & forums

Qualifications

  • College diploma in Social Services (minimum)
  • Bachelor of Social Work preferred

Experience and Skilled Knowledge Requirements:

  • Minimum experience of 2 to 3 years of related experience required
  • Previous experience with people experiencing or at risk of homelessness
  • Thorough knowledge and understanding of, and a commitment to, anti-racism, anti- oppression and equity are required
  • Knowledge and understanding of Mental Health/Addictions
  • Knowledge of the Tenant Protection Act

Skills and Capabilities:

  • Problem solving skills are essential
  • Strong conflict resolution and mediation skills within a solution-focused frameworkThe responsibilities associated with this position demand a high degree of confidentiality
  • Excellent oral and written communication skills (Processing/Desktop, Publishing, Spreadsheet/Graphing, Database computer skills essential)
  • Ability to work independently as well as part of a team
  • Must have good interpersonal/time management skills
  • Good knowledge of community resources
  • Valid Driver’s License is a requirement

Successful candidate will be required to provide upon hiring:

  • A clear vulnerable sector screening
  • A clean drivers abstract
  • Participate in our online Armatus Abuse Training and Health and Safety training required upon hiring, as well as updated annually

HOURS: 35 hours per week. Ability to work on weekends and evenings to meet client needs
